Playing with this flag will shuffle gear effects. The tier of the item is taken into account.
When gear is randomized, all special effects (such as the gear granting the Shield status to its user or a boost of Magic+2) are removed, then the randomizer rolls a new effect (which might be nothing, as many pieces of equipment don't have boosts or buffs) for each piece of gear.

Healing Item randomization is a separate flag.
A boost is defined as a bonus effect on an item that increases one of a character's stats. For example: Speed +1 is a boost, as is Magic Defense +10.
A buff is defined as a positive status effect given by an item to its wearer. For example, Haste or the Shield effect are buffs.
A weapon effect can either grant a(nother) boost, or it can be an extra effect that is executed when the weapon is used - such as 50% HP damage (the Graedus effect) or a 60% chance to inflict a status effect on the enemy.
Items that have been randomized will have their names and descriptions reflect those changes. The following information is for generalized renaming. Certain things (like awesome armor or ultimate weapons) have special renaming rules which can be found on the individual pages linked above.
If an item is changed by the randomizer, it will get a +, -, or ? at the end of its name. An item can have a Boost and/or an effect.
A boost is considered better (+) if it affects the same stat (or more stats) and has an equal or larger number, and worse (-) if it affects the same stat (or fewer stats) and has an equal or smaller number. Otherwise, it's unknown (?).
For example, compared to a hypothetical Power/Stamina+2, Power/Stamina/Speed+2 or Power/Stamina+3 would be better, Power/Stamina+1 or Power+2 would be worse, and Speed+1 would be unknown.
An item is treated as “better” if no effect was replaced with an effect (+); “worse” if an effect was replaced with no effect (-); “unknown” (?) if the effect was swapped for another.
The following armor pieces do NOT have their effects randomized (they MAY be affected by boosts):
